Acupuncture & Chinese Medicine for Fertility

Acupuncture & Chinese Medicine for Fertility: What the Research Says

Fertility care is one of the areas where Western medicine and Traditional Chinese Medicine (TCM) are increasingly being used alongside one another.

And this is an exciting area of research.

As more people turn to acupuncture as a complementary therapy during fertility treatment, researchers have been investigating an important question:

Can acupuncture help support fertility and improve outcomes during IVF and embryo transfer?

The answer isn't quite as simple as yes or no. The research is evolving, and results vary depending on the timing, frequency and type of acupuncture used. But there are some promising findings—and there is growing interest in how individualized acupuncture may fit into modern fertility care.

From the Paulus Protocol to Individualized Acupuncture

One of the studies that brought significant attention to acupuncture during IVF was published in 2002 and became known as the “Paulus Protocol”.

The study looked at acupuncture performed around the time of embryo transfer and reported a clinical pregnancy rate of approximately 42.5% in the acupuncture group compared with 26.3% in the control group.

These findings helped spark considerable interest in acupuncture as a complementary treatment during assisted reproductive technology.

However, fertility research has continued to evolve since then.

And one of the most important developments has been a greater focus on live birth rates, individualized treatment and the timing and dosage of acupuncture.

What Does More Recent Research Tell Us?

A 2022 retrospective study by Phillippi and colleagues examined individualized acupuncture provided immediately before and after frozen embryo transfer (FET).

The study included 579 FET cycles in which patients received individualized acupuncture and 1,751 FET cycles in which patients elected FET without acupuncture. The researchers looked specifically at live birth outcomes.

The acupuncture treatment was individualized rather than following one standardized protocol, with acupuncture provided for approximately 30 minutes before and immediately after embryo transfer.

The researchers reported that individualized acupuncture was associated with higher live birth rates and fewer biochemical pregnancies compared with FET alone.

This distinction matters when we're talking about fertility.

And the research hasn't stopped there.

A 2024 systematic review and meta-analysis examined 38 randomized controlled trials involving nearly 6,000 participants and found that the timing of acupuncture appeared to matter. In frozen embryo-transfer cycles, acupuncture administered before FET was associated with higher clinical pregnancy and live birth rates. The researchers also found that treatment dosage may influence outcomes.

At the same time, other recent reviews have found more modest effects and have emphasized that the overall certainty of evidence remains limited because studies use very different acupuncture protocols, control groups and treatment schedules.

So what does this mean for someone going through fertility treatment?

It means there is promising research—but acupuncture should be viewed as a complementary therapy, not a guarantee of pregnancy or a replacement for medical fertility treatment.

Why Does Individualized Treatment Matter?

This is where the philosophy of Traditional Chinese Medicine becomes particularly interesting.

In TCM, there isn't necessarily one fertility treatment that is appropriate for everyone.

Two people can both be preparing for an embryo transfer and receive completely different acupuncture treatments.

Why?

Because we look at the whole person.

Your menstrual cycle, sleep, digestion, stress levels, energy, pain, emotional wellbeing and overall health may all be considered when developing your treatment plan.

Rather than asking only:

“What fertility diagnosis do you have?”

TCM also asks:

“What is happening in your body right now?”

And that is one of the things I love most about incorporating Chinese medicine into fertility care.

How Might Acupuncture Support Fertility?

Research into acupuncture and fertility has explored a number of potential mechanisms and outcomes.

Supporting the nervous system

Fertility treatment can be physically and emotionally demanding.

Acupuncture may help support relaxation and nervous-system regulation, which can be particularly valuable during periods of stress and uncertainty.

Supporting blood flow

Researchers have investigated acupuncture's potential effects on blood flow and vascular regulation, including blood flow to reproductive tissues.

This is one of the proposed mechanisms behind acupuncture's use around fertility treatment, although improved blood flow does not automatically mean improved pregnancy or live birth rates.

Supporting menstrual cycle regulation

From a TCM perspective, menstrual health is an important part of fertility care.

Acupuncture may be incorporated into treatment plans for concerns such as:

* Painful periods

* PMS

* Irregular cycles

* Cycle-related stress

* Ovulatory concerns

Supporting the IVF experience

Acupuncture is also being studied as an adjunct to IVF and FET, including during ovarian stimulation and around embryo transfer.

Recent systematic reviews suggest that timing and treatment frequency may matter, although there is still no universally established acupuncture protocol for fertility treatment, which is good, because it should be tailored to your individual needs.

Supporting overall wellbeing

Sometimes the most important part of fertility care isn't another test, medication or procedure.

It's having a place where you can slow down.

Acupuncture can provide an opportunity to rest, regulate, reconnect with your body and receive individualized care while continuing with your medical fertility treatment.

What About Starting Acupuncture Before IVF or FET?

This is a question I hear often:

“Do I need acupuncture right before my embryo transfer, or should I start earlier?”

While much of the public conversation around fertility acupuncture focuses on treatment immediately before and after embryo transfer, research has also looked at acupuncture administered earlier in the IVF process.

A 2024 meta-analysis found that treatment timing and dosage may influence outcomes, with some benefits seen when acupuncture was started before or during particular stages of IVF/FET treatment.

From a TCM perspective, starting earlier can also allow more time for individualized treatment based on your cycle, symptoms and overall health.

For someone preparing for IVF or FET, this may mean beginning treatment several weeks—or even a few months (at least 3 months before IVF or FET, ideally)—before the procedure rather than waiting until transfer day.

This doesn't mean that everyone needs months of acupuncture, or that starting earlier guarantees a better outcome.

Rather, it gives us more time to work with you as a whole person, rather than focusing solely on the few days surrounding embryo transfer.

The Bottom Line

The research surrounding acupuncture and fertility is promising—but it is also nuanced.

Some studies and meta-analyses have reported improvements in clinical pregnancy and live birth outcomes. Differences in treatment protocols, timing, dosage and study quality make it difficult to say that one specific acupuncture protocol works for everyone.

And this is where I believe Chinese medicine has something valuable to offer.

Your fertility journey is not just about an embryo, a hormone level, an ultrasound or a lab result.

It's about you.

Your body.

Your cycle.

Your sleep.

Your stress.

Your energy.

Your emotional wellbeing.

And everything you're carrying while going through this process.

Chinese medicine gives us the opportunity to look at all of those pieces together and create a treatment plan that is individualized to you—not a one-size-fits-all protocol.

If you're preparing for IVF, FET or trying to conceive naturally, acupuncture can be incorporated alongside your existing medical care to support you throughout the process.

You don't have to choose between Eastern and Western medicine. There can be value in bringing the two together.
